Terminal Point Technique (TPT)

Understanding Drop-Table Chiropractic Adjustments

TPT, commonly associated with the Thompson Technique, is a chiropractic approach that uses a purpose-built treatment table with movable sections.

At Hayek Chiropractic Care, drop-table adjustments are among the manual therapy techniques that may be considered when managing certain musculoskeletal presentations.

The table allows a chiropractor to apply a controlled manual impulse while a supported section moves a short distance. Whether this approach is appropriate depends on your symptoms, clinical assessment and preferences.

Terminal Point Technique

Terminal Point Technique is a method of chiropractic manipulation associated with the work of chiropractor J. Clay Thompson.

The technique uses a chiropractic drop table with padded sections that can be raised slightly and released during an adjustment.

As the chiropractor applies a controlled manual impulse, the relevant table section moves downward a short distance and stops at a predetermined position.

How Is the Drop Table Used?

Different sections of a chiropractic drop table may be used depending on the patient's presentation, the equipment available and the technique being considered.

Head & Neck

Our drop tables have an adjustable headpiece that may be used when performing certain cervical spine techniques.

The chiropractor positions and supports the head and neck while applying a controlled movement, where clinically appropriate.

Not all neck symptoms require manipulation, and the choice of technique depends on individual assessment findings along with preference.

Mid-Back (Thoracic Spine) & Ribs

Drop-table sections may be used when applying certain manipulative techniques around the thoracic spine and ribs.

The patient is supported on the table while the chiropractor uses a controlled movement directed towards the selected region.

Lower Back (Lumbar Spine)

Drop-table techniques may also be applied around the lower back.

The table provides a supported surface while the chiropractor delivers a controlled manual impulse.

For lower back pain, hands-on treatment is only one possible management option and may be considered alongside education, exercise and advice about maintaining activity.

The drop tables include sections designed to support techniques involving the pelvic and sacroiliac regions.

Drop-table sections can also be used for some joints of the arms and legs.

How Does TPT Fit Into Evidence-Based Care?

Spinal manipulation is one of several approaches used to manage musculoskeletal conditions, including lower back pain. Research indicates that spinal manipulation can improve pain and function in some people, with outcomes generally comparable to other conservative treatments.

Terminal Point Technique (TPT) is a form of spinal manipulation that uses a drop table to deliver controlled adjustments.

At Hayek Chiropractic Care, treatment decisions are guided by clinical assessment, individual needs and patient preferences. Where appropriate, TPT may form part of a broader management plan that includes education, exercise and practical advice to support movement and everyday activities.

A 2019 systematic review in the BMJ (Rubinstein et al.) found spinal manipulation had effects similar to other recommended treatments for chronic low back pain.
